My house is sitting on concrete blocks, and over the course of a couple of years has become uneven. This is evident by the cracks on the walls and because doors and windows no longer close correctly. I'm going to do the job on my own and don't have many tools to work with. I'd like to know how to do it in an accurate way.

Well its clear that something has failed.
With out a picture, you need to know if the concrete blocks failed or the ground/concrete block interface failed. Just a guess but it sounds like the piers (concrete blocks ) are undersized for the weight that it supports.
I would accurately measure which are uneven and by how much. I would try to get all points with in 1/2 inch of each other.
Tools needed. you can get a lazer level at Lowes for about 30 bucks. I would rent the rest (jacks, cement mixer, etc).
If I had just one point out of whack, then the solution is very easy. Jack up the house, fix the concrete then lower the house.
If more points are uneven then the solution becomes more complected.
I would also talk to local house movers or some one locally that has knowledge about the soils and how much they will support...

Do you any idea how the building got out of whack? is foundation still level or have the logs moved on the pins?
